Fall Forms
- Official Registration Form this form needs to be filled out in order to participate in classes
- Policy Agreement 2026-2027 this form must be filled out and turned in with your official registration form
The below form is not mandatory, however, if you would like to participate it must be completed.
- ACH Form 2026–This form allows a family’s tuition to be taken automatically out of your bank account or you may keep a credit card on file to be used to pay tuition. This form must be printed off and turn in with the proper documentation by the 15th of each month to be able to be used for the month to follow.
